Thrombotic thrombocytopenic purpura (assessment and treatment selection) is a blood disorder that occurs when the immune system mistakingly destroys platelets (blood clotting cells). Therapy aims to halt this process and prevent dangerous bleeding. The choice of treatment strategy depends on the severity of platelet count drop, bleeding, and how long it lasts. First-line therapy is based on glucocorticosteroids, which are the immunosuppressive drugs that can relatively quickly raise platelet levels. If the response to steroids is insufficient or platelet levels fall again, other methods may be recommended. The decision to initiate treatment and select an approach is taken individually, taking into account the severity of the condition, the patient’s age, concomitant diseases, and lifestyle.
